Prior to starting services, families are typically asked to complete: Demographic packet Insurance information (if applicable) Relevant records (diagnostic reports, evaluations, IEPs, or treatment history, if available) Treatment consent forms Our team will guide families through the process and help make paperwork as straightforward as possible.

The intake and authorization process varies depending on factors such as insurance requirements and authorizations, availability of documentation, and scheduling between provider and families. We strive to move the process along efficiently and keep families informed every step of the way.
